Introduction

The 2010 Clinical Practice Guidelines for the Diagnosis and Management of Osteoporosis in Canada and the 2015 Clinical Practice Guidelines for the Frail Elderly offer evidence-based screening and treatment recommendations for adults over 50 years. We have developed an osteoporosis and falls assessment tool based on the guidelines, that can be integrated into the electronic medical records (EMR) with the aim of improving osteoporosis-related care in family practice.

This tool is now available for PS Suite EMR users and is currently being developed for OSCAR EMR users. The custom form is multifunctional and offers a number of features to support family physicians in their decision making process during osteoporosis patients’ visits.

     
 
     
     
     

When can you use this form:

  1. Screening patients without a previous diagnosis of osteoporosis
  2. Reassessing patients for osteoporosis follow up
  3. Reassessing patients with a fall and/or fracture related events
 

Suggestions for use:

  • Current patient needs Bone Mineral Density (BMD) test
  • Assessment/re-assessment of patients ≥ 65 years
  • Reassessment of patients on osteoporosis treatment
  • Patients 50 and over with a fragility fracture
     
     
     
 
     
     

Disclosure: This form was developed by the Geriatric Education & Research in Aging Sciences (GERAS) Centre and McMaster University, in partnership with the Ontario College of Family Physicians, Osteoporosis Canada, and the Hamilton Family Health Team’s Quality Improvement Unit. STEP 1: Osteoporosis Self-Learning Education

The machealth.ca osteoporosis e-module is a self-learning educational module that will teach you the 3 secrets to practical and evidence-based approach to osteoporosis. The goal of the osteoporosis e-module is to improve the assessment and recognition of osteoporosis while improving treatment and management of osteoporosis of those at risk. When completing the module you will be eligible to receive credits.

The program has been accredited by McMaster Continuing Health Science Education for up to 1.5 Mainpro-M1 credits. The education program is also approved as an Accredited Group Learning Activity under Section 1 of the Framework of CPD options for the Maintenance of Certification Program of the Royal College of Physicians and Surgeons of Canada and approved by McMaster Continuing Health Science Education for up to 1.5 hours.

Registration is quick, simple and free!  You may access the osteoporosis e-module at anytime by following these steps:

  1. Access machealth.ca at https://machealth.ca/programs/osteoporosis/
  2. Log in – if you are not a member, please sign up (click on “sign up now”). Note: You must be a member in order to access the module and/or receive credits upon completion of the module. Registration is quick and simple.
  3. Complete the module at your leisure, preferably before the educational teleconferences take place.
